One (1) Proof of Grade 12 Standing/Graduate (e.g. School Card, Principal’s Certification, or Diploma)
One (1) Proof of Residency (e.g. Punong Barangay’s Certification, Voter’s ID, PhilSys National ID, or Postal ID)
